How long does this cover?

Sunday Sandlot (the "Program"), a free, volunteer-supported baseball program for ages 9–11, and Texas Standard Baseball (the "Facility Operator"), which provides the premises at 2234 Rabb Rd, Fresno, Texas 77545 and coaching support only — together with their respective owners, members, managers, partners, officers, directors, employees, coaches, instructors, volunteers, agents, independent contractors, sponsors, affiliates, landlords, lessors, property owners, insurers, successors, and assigns (the "Released Parties").

Every Sunday Sandlot session my player attends through December 31 of this year.

Keeping the participant safe

Optional. Tell the coaches anything they would need to know on the field — an allergy, asthma, a medication, or a recent injury. Leave it blank if there is nothing.

Anything we should know (optional)

We deliberately do not ask for a policy number, insurance carrier, or physician. Nobody needs them to treat a participant in an emergency — a policy number is for billing — so we do not collect or store them.

Questions parents ask

Who has to sign the Sunday Sandlot waiver?: A parent or legal guardian signs the participant agreement for every player before their first Sunday.
How long does signing take?: About a minute. The form is a guided set of steps built for a phone, and it saves your progress as you go.
Can I sign for more than one child?: Yes. You can add another child inside the form, and each child gets their own participant ID and their own signed PDF.
Do I get a copy of the signed agreement?: Yes. A PDF copy is emailed to the guardian address you enter, and it is also saved to your profile.
Is the waiver good for the whole season?: You can sign for a single session date or for the full season — choose the term at the start of the form.
